To participate in the program, your practice/clinic is required to offer Neurofeedback services.

Program Guidelines and Provider Agreement:
In becoming a provider for this program, you agree to offer free Neurofeedback services to veterans. Additionally, you warrant that your office has certified or licensed health care practitioners who meet the requirements for health related services in your state. The number of veterans you choose to see at one time is at your discretion based on the resources available in your practice or at your clinic. At the EEG Institute, we limit ourselves to one veteran at a time. Once they complete the program, we offer free Neurofeedback services to the next veteran on the waiting list.

If waiting for an opening is not an option for a veteran, we offer Neurofeedback services to that person at a discounted rate.

*You may of course offer services in any configuration that is reasonable for you, as long as you offer to work with at least one free veteran at a time at your office.

**If you wish to discontinue offering these services to veterans at a minimum of one at a time, you agree to notify us so we can remove your listing.
